This is an original copy of the Florida DS License Curriculum that you need to provide to the Florida Division of Licensing when your license is up for renewal.

NOTE: The Division requires that you provide them the following:

• The list of the instructors teaching at your school, which should match the list registered with the State if you did an Employee Action Report (EOR) as required by law.
• A copy of your school examination material:
• Final examination;
• Answer sheets;
• Answer key.
• A copy of your Student Certificate.
• A copy of your curriculum, yes EVEN if you just submitted it as part of coming into compliance, which is what ordering this curriculum will provide you with.
